Part of the magic of Santa comes from his seemingly boundless powers of memory, especially when he can remember a child’s name or “predict” their wish list — even know their teacher’s name!

The form below is designed to be filled out by parents prior to Tall Santa’s visit (and after you have booked a visit). One form for each child in attendance should be submitted at least five days before the visit. Enter all information you are comfortable with sharing. This information is kept confidential and not shared with anyone other than Santa. All data is deleted after completion of the visit.
